Spay/Neuter and Microchip Agreement

If the pet has not yet been spayed or neutered by Lagacy Paws Rescue, I agree to have the adopted pet spayed or neutered by the age of 6 months, unless otherwise advised by a licensed veterinarian for health reasons.*
